Please find below information in regards to ONE-T Plan Eligibility, especially as it relates to eligibility by employment status and common leaves of absence.  Included in the attached document is an overview of common employment statuses and how coverage works:

Important Note re: Long Term Disability:

  ONE-T will continue to cover your benefits costs for a maximum of 24 months (2 years) following the date of the approved disability claim. If you have been on LTD or WSIB leave for more than one year at the transition date of June 1, 2018, ONE-T will continue coverage for a maximum of one year following the transition date.  If you wish to continue your benefits coverage beyond that period, coverage would be 100% member-paid.
